Section 9.5-406 - Applications for licenses(a) An applicant for a license shall submit an application to the Department on the form that the State Superintendent requires.(b) An application for a license shall contain:(1) The name of the applicant;(2) The proposed location of the child care center;(3) The name of the individual to be in charge of the child care center; and(4) Any other information that the Department requires.Renumbered by 2016 Md.
