(A) Every health maintenance organization subject to this chapter shall pay to the department the following fees: (1) for filing an application for a certificate of authority, two thousand dollars;(2) for filing an amendment to the organization documents that requires approval, one hundred dollars;(3) for filing each annual report, one thousand dollars;(4) for transferring a certificate of authority from one entity to another which qualifies for such a certificate of authority, two thousand dollars.(B) Fees charged under this section must be deposited in the general fund of the state.
