Section 600.6 - [Effective 7/1/2024] Attachments to an adoption petitionAn adoption petition shall have attached to it the following:
1. A certified copy of the birth certificate showing parentage of the person to be adopted or, if such certificate is not available, a verified birth record. The department shall provide a certified copy of a birth certificate or a verified birth record, as applicable, to the person adopting a child when the department is the guardian of the child.2. A copy of any order terminating parental rights with respect to the person to be adopted.3. If parental rights were terminated pursuant to chapter 232, a copy of any court orders concerning whether ongoing contact between siblings not placed with the person to be adopted is in the best interest of each sibling.4. Any written consent and verified statement required under section 600.7, except the consent required under subsection 1, paragraph "d", of that section.5. Any preplacement investigation report that has been prepared at the time of filing pursuant to section 600.8.6.
