1.a. The secretary of state may prescribe and furnish on request any of the following forms: (1) An application for a certificate of existence or certificate of registration.(2) A foreign corporation's registration statement.(3) A foreign corporation's statement of withdrawal.(4) A foreign corporation's transfer of registration statement.(5) The biennial report required by section 490.1621.b. If the secretary of state so requires, use of the forms provided in paragraph "a" is mandatory.2.
