Cal. Wat. Code § 39931

Section 39931 - Ballots

On the ballots provided for the formation election shall be printed:

(a) The words "Water storage district-Yes" and "Water storage district-No," or words equivalent thereto.

(b) The names of persons to be voted for at the election.
