Section 08.62.090 - Application(a) A person who desires to be licensed under this chapter shall apply in writing to the department.(b) The application shall provide the information and be made on a form prescribed by the department.(c) In order to be eligible to take the next scheduled examination, a person shall file the application with the board at least 60 days before the date of the examination.
