(1) Legal papers shall be accompanied by an Orphans' Court Cover Sheet as set forth in the Appendix to these Rules.(2) Petitions for Adjudication/Statements of Proposed Distribution filed in accordance with Pa. O.C. Rule 2.4 and Phila. O.C.
