A motion to withdrawal an attorney's appearance shall include (1) the name, address, and telephone number of the defendant; (2) the date of birth of the defendant and the last four numbers of his or her Social Security Number; (3) if the defendant is incarcerated, the name of the facility and his or her inmate identification number; (4) a list of prior counsel who represented the defendant in the case; and (5) a list of codefendants and the name of any other defendant who is charged in the same incident, including the name(s) of counsel for any other defendant identified. If the appointment of new counsel is required to continue the case, the motion to withdraw should include a request for appointment of new counsel.
