I verify that the foregoing is true and correct to the best of my knowledge:
1. I understand that I have the right to be represented by counsel. I am represented by counsel of record and wish to waive my appearance at formal arraignment.2. My attorney concurs in this waiver.3. I understand the nature of all charges against me.4. I am aware of my right to file motions, including a request for a bill of particulars, a motion for pre-trial discovery and inspection, a motion requesting transfer from criminal proceedings to juvenile proceedings pursuant to 42 Pa. I understand that I am required to report for future court proceedings including my plea date and date for trial on the date and time provided to my attorney.
