A defendant against whom a default judgment is entered in a violation matter may file a request for relief from default judgment within a reasonable time, not to exceed one (1) year. A request for relief must be in writing and set forth facts which demonstrate that the failure to appear or to exercise one's options described in SLR 16.001, was due to mistake, inadvertence, surprise or excusable neglect.
