Pursuant to ORS 153.080, if a signed waiver (Appendix A) is filed by the alleged violator, testimony in a trial of any violation shall be allowable by affidavit or by declaration under penalty of perjury. A copy of the Witness' affidavit or declaration shall be provided to the alleged violator before trial.
