When appointing attorneys as counsel to represent an indigent defendant in a capital case pursuant to Appt.Coun.R. 5.02 or 5.03 for which the county will apply to the Ohio Public Defender Commission for reimbursement of costs, the court shall require the defendant to complete a financial disclosure form. The court shall follow Ohio Adm.Code 120-1-3as a guideline to determine indigency and standards of indigency.
