All pleadings, motions, briefs and other papers filed with the clerk of the court of appeals shall be legibly typewritten or printed on 81/2 x 11 paper. Every filing shall contain the appropriate case caption; attorney's name and Supreme Court registration number; attorney's firm, if any, and office address; attorney's telephone number and email address; or, if a party is not represented by counsel, every filing shall contain the appropriate case caption, the party's printed name, street address, email address, and telephone number. In all filings, a blank space of at least four (4) inches shall be left at the top of the first page for endorsements thereon.
