A motion for the termination of parental rights shall be substantially in the form approved by the Supreme Court.

ANNOTATIONS The 2008 amendment, approved by Supreme Court Order No. 08-8300-042, effective January 15, 2009, changed "commencement of proceedings" to "form of motion" in the title; deleted former Paragraph A which provided for the commencement of a termination of parental rights proceeding; deleted former Paragraph B which provided for the joinder of parents as parties to a termination of parental rights proceeding; deleted the letter designation and title of former Paragraph C; and changed "motion or petition" to "motion for the termination of parental rights".
