Rule 89 - Form of Affidavit(a) The affidavit of small claims complaint must contain:(1) a statement that the defendant is indebted to the plaintiffin a specified amount;(2) a brief summary of the basis of the indebtedness; and(3) a statement that the small claims court has jurisdiction over the case pursuant to NRS 73.010.(b) The affidavit of small claims complaint must include: (1) an order for the defendant to appear and be prepared to answer the claim on the date and time of the trial; and(2) a notice to the defendant that failure to appear at the date and time of the trial may result in the entry of a judgment against the defendant.Nev.
