Rule 4-204.1 - Notice of Investigation(a) A Notice of Investigation shall accord the respondent reasonable notice of the charges against him and a reasonable opportunity to respond to the charges in writing. The Notice shall contain:(1) a statement that the grievance or written description persuant to Bar Rule 4-202(a) is being transmitted to the State Disciplinary Board;(2) a copy of the grievance or written description persuant to Bar Rule 4-202(a);(3) a list of the Rules that appear to have been violated;(4) the name and address of the State Disciplinary Board member assigned to investigate the grievance and a list of the State Disciplinary Board members; and(5) a statement of the respondent's right to challenge the competency, qualifications or objectivity of any State Disciplinary Board member.(b) The form for the Notice of Investigation shall be approved by the State Disciplinary Board.(c) The Office of the General Counsel shall cause the Notice of Investigation to be served upon the respondent pursuant to Rule 4-203.1 .Ga. R. Prof. Cond. 4-204.1
