Prior to, or at the start of, any substantive hearing as required by A.R.S. § 8-221(J), the guardian ad litem or attorney for a child shall provide the court with information regarding the most recent meeting or meetings with the child. The information provided shall be in writing and include, at a minimum, the date of the meeting, the general location of the meeting, the duration of the meeting, who was present at the meeting, and whether the guardian ad litem or attorney for the child had sufficient interaction with the child to adequately advocate on behalf of the child. The court shall provide a sample format to a guardian ad litem or an attorney for a child upon request.
