The field office manager or designee shall review all court orders on placements to ensure that they contain the following information:
(a) Custody of the child;
(b) The child's school district or school districts of residency;
(c) A statement to the effect that continuation of residence at home is contrary to the welfare of the child or that placement is in the best interests of the child; and
(d) A statement that either reasonable efforts were made to prevent removal from the home, or it was not appropriate or in the best interests of the child to prevent removal from the home.
If a court order is incomplete, he/she shall immediately contact the court to resolve the issues.
