Any person wishing to take an examination shall make application therefore on the approved form (See Appendix A). The application shall be submitted to the Board together with the $10.00 examination fee (checks payable to Board). The Board shall then notify the applicant and the designated proctor of the time and place for examination and shall furnish the proctor with the examination questions. The proctor will administer the examination in accordance with directions of the Board and will thereafter return all papers to the Board, together with his report on the examination (See Appendix B).
