Services for which the Board charges a fee shall include, but is not limited to, the following fee schedule:
| (a) | Application Fee | $200.00 |
| (b) | Renewal Application Fee | $200.00 |
| (c) | Late Renewal Application Fee (June 15 to June 30) | $225.00 |
| (d) | Reinstatement Fee (July 1 to September 30) | $300.00 |
| (e) | Reissuance Fee (lapsed license less than 5 years) | $400.00 |
| (f) | Return to Practice Fee (lapsed license more than 5 years) | $400.00 |
| (g) | Relicensure Following Discipline Fee | $500.00 |
| (h) | Non-sufficient Funds Fee | $30.00 |
| (j) | Roster Fee | $50.00 |
| (k) | Replacement Wall Certificate | $10.00 |
| (l) | Replacement Pocket ID Cards | $10.00 |
| (m) | License Verification Fee | $15.00 |
| (n) | Copy Fee | $.25 per page |
