Section 64-4 - Central Visual Acuity Disability Permit Qualifications

Any person having a central visual acuity disability as attested in writing by the applicant and certified by a medical doctor, nurse practitioner, physician assistant, optometrist or ophthalmologist licensed to practice medicine, on a form provided by the Department, may be issued a Central Visual Acuity Disability Permit.
