Section 6-7-4 - Fee schedule

4.1Initial License/Registration Fees (Non-Refundable)

4.1.1. Main funeral home	$625.00
4.1.2. Branch funeral home	$440.00
4.1.3. Crematory	$440.00
4.1.4. Funeral services licensee	$200.00
4.1.5. Funeral director license	$200.00
4.1.6. Apprentice	$220.00
4.1.7. Crematory operator	$150.00
4.1.8. Courtesy card	$375.00
4.1.9. Mortuary service	$500.00

4.2.The following fees are for biennial renewal fees and are non-refundable.

4.2.1. Main funeral home	$500.00
4.2.2. Branch funeral home	$345.00
4.2.3. Crematory	$440.00
4.2.4. Funeral services licensee	$250.00
4.2.5. Funeral director	$250.00
4.2.6. Apprentice	$220.00
4.2.7. Crematory operator	$150.00
4.2.8. Courtesy card	$375.00
4.2.9. Mortuary services	$500.00
4.2.10 Inactive license	$50.00
4.2.11. 6.2. Reactivation of Inactive License	$12.00 plus biennial renewal fee.

4.3Late fees

4.3.1. The following fees will be assessed according to the renewal date.

4.3.1.a. Late fee on or after July 1 of the renewal year	$190.00 plus the renewal fee.
4.3.1.b. Reinstatement fee after one (1) year	$350.00 plus the current renewal fee.
