Section 19-3-7 - Change of Name and/or Address7.1. If a licensee legally changes his or her name through marriage, divorce court order or other means, he or she shall send this information to the office of the board. The information shall include both the full prior name and the new name, in a properly executed affidavit or a certified copy of the marriage certificate or divorce decree. The licensee shall submit an application and these documents to the nurse portal on its website at wvrn.boardsofnursing.org.7.2. A licensee shall notify the board of any change in residence or mailing address within 30 days of the change. This notification shall be submitted in writing to the via the nurse portal.
