Section 141-83-7 - The applicant will take the following action

a. AGO Form 600-200-5, Application for Tuitional Assistance must be completed in its entirety and endorsed by the unit commander. The form is then forwarded to TAG, WV ATTN: WVEEP. Upon receipt of the properly executed application, part three will be

completed by this headquarters. If the request is approved, tiie certified application will be re turned to the individual guardsmen home address. A separate application must be submitted for each semester.

b. The approved application will be presented to the institution by the individual at the time of registration.

c. If the certificate is not used for the semester for which it is authenticated, it must be rettuned immediately for cancellation.

d. Payment will be made each semester directly to the institution concerned. In the event the individual is required to make payment prior to receipt of approved application, reimbursement may be made upon presentation of paid receipt for tuition and fees to TAG, WV ATTN: WVEEP.
