Section 388-845-1197 - What are the limits for person-centered plan facilitation?(1) Support needs for person-centered planning facilitation are limited to those identified in the waiver participant's DDA assessment and documented in the person-centered service plan.(2) Person-centered plan facilitation may include follow up contacts with the waiver participant and his or her family to consult on plan implementation.(3) An IFS waiver participant's person-centered plan facilitation is limited to the participant's annual allocation.(4) A CIIBS waiver participant's person-centered plan facilitation must not exceed the CIIBS combined specialized hourly services allocation of $5,000 per year.Wash.
