Section 388-845-3061 - Can a change in your person-centered service plan be effective before you sign it?

If you verbally request a change in service to occur immediately, DDA can sign the person-centered service plan and approve it prior to receiving your signature.

(1) Your person-centered service plan will be mailed to you for signature.
(2) You retain the same appeal rights as if you had signed the person-centered service plan.
