Section 388-828-1345 - When will I receive the services for which I have been determined eligible?

You will begin receiving a service for which you have been determined eligible after:

(1) Your DDA assessment is complete;

(2) You are found financially and functionally eligible for services;

(3) You have given consent for services and approved your plan of care;

(4) You have chosen a qualified provider; and

(5) DDA has authorized the qualified provider to provide services.
