Section 388-473-0080 - Medically related items or services as an ongoing additional requirement(1) We authorize the following benefits as a one-time payment within a 12 month period: (b) Optometrist visit for eye glasses;(2) Benefits are issued after we determine: (a) You did not qualify for the service or item from any state, federal, or private insurance coverage; or(b) You have been unable to obtain a replacement through state, federal, or private insurance.Wash.
