Section 388-112B-0080 - What documentation is required for successful completion of geriatric behavioral health worker training?Geriatric behavioral health worker training must be documented by a certificate, transcript, or proof of successful completion of training issued by a qualified instructor that includes:
(1) The name of the student;(2) The title of the training as approved by the department;(3) The number of hours of the training;(4) The name of the facility providing the training;(5) The instructor's name;(6) The instructor's signature or other authorized signature from the training entity; and(7) The completion date of the training.Wash.
