Section 388-107-1090 - License valid and not transferable(1) The enhanced services facility is required to renew the license each year.(2) The license remains valid unless: (a) The department takes enforcement action to suspend or revoke the license per law;(b) The facility voluntarily surrenders the license and closes the facility;(c) The facility relinquishes the license; or(d) The facility fails to pay the annual licensing fee.(3) The facility license is: (a) Not transferable; and(b) Valid only for the provider and address listed on the license.Wash.
