Section 388-101D-0460 - Monitoring physical and mechanical restraints(1) The service provider must ensure that any client who is being physically or mechanically restrained is continuously observed to ensure that risks to the client's health and safety are minimized.(2) The service provider must keep documentation that includes:(a) A description of events immediately preceding the client's behavior which led to the use of the restraint;(b) The type of restraint used;(c) Length of time the client was restrained;(d) The client's reaction to the restraint;(e) Staff that were involved; and(f) Injuries sustained by anyone during the intervention.Wash.
