Section 308-15-087 - How do I reactivate my inactive license?(1) If you are returning to active status from less than five years of inactive status, submit to the board:(a) A letter of application requesting reactivation of your license; and(b) The current renewal fee.(2) If you are returning to active status after five years of inactive status, submit to the board: (a) A letter of application requesting reinstatement; and(b) The current renewal fee plus the late penalty fee.Wash.
