Any interested party, as defined in RCW 39.12.010(4) may file with the department a complaint alleging a violation of the prevailing wage laws. The complaint must describe the alleged violation and identify the alleged violator. It would aid the department's investigation if the complaint also specifies:
(1) The name and address of the complainant;(2) The address of the alleged violator;(3) The name and address of the public agency that awarded the contract;(4) The date the public agency accepted the completed public work (if applicable);(5) The specific rates of wages paid by the violator and the rates that allegedly should be paid;(6) The exact amount of prevailing wages that are alleged to remain unpaid; and(7) The date the bids were due on the public works project.Wash.
