Section 173-408-150 - CertificationAny application form, report, compliance certification, or other information submitted pursuant to this chapter shall contain the following written certifications made and signed by the person making the submission:
(1) "I certify under penalty of perjury under the laws of the state of Washington that I am duly authorized to make this submission on behalf of the party that is required to provide the information contained therein pursuant to Chapter 173-408 WAC."(2) "I certify under penalty of perjury under the laws of the state of Washington that, based on information and belief formed after reasonable inquiry, all statements and information contained in the submitted document are true, accurate, and complete."Wash.
