Section 8VAC20-780-150 - Form and content of immunizations and physical examination reports for childrenA. The current form required by the Virginia Department of Health or a physician's form shall be used to report immunizations received and the results of the required physical examination.B. Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department.8 Va.
