Section R990-100-12 - Financial Reports and Reimbursements(1) Financial reports are to be submitted on a no more frequently than monthly and no less frequently than quarterly basis.(2) Each eligible entity shall receive reimbursement based on a monthly financial status report and certification of work program activities.(3) Each report must be signed by either: (a) the contract signatory, or(b) someone designated by the signatory, with a letter of designation filed with the state.(4) Final reimbursement requests shall be due by the date stipulated by SCSO contract.Utah Admin.
