Section R649-8-12 - Form 11, Monthly Oil and Gas Disposition Report1. All oil and gas well operators shall complete this form monthly to account for all oil and gas dispositions from each entity.1.1. The report should account for the physical dispositions of all oil and gas produced during the report month from each well or group of wells (entity).1.2. Only the initial disposition of each product as it leaves the well site or is used at the well site should be reported.1.3. Residue gas and/or load oil received from another well, plant, or field should not be shown on this report.2. This report shall be submitted in conjunction with Form 10, Monthly Oil and Gas Production Report and Form 12, Report of Transferred Oil on or before the fifteenth day of the second calendar month following the month of production.Utah Admin.
