Section R590-91-5 - Policy Forms and Reserves(1) Credit insurance shall be issued in a form described in Section 31A-22-803.(2) Each policy form, certificate, notice of proposed insurance, application, endorsement, and rider to be delivered or issued for delivery in this state shall be filed with the commissioner as required by Sections 31A-21-201, 31A-22-807, and 31A-22-808.(3) Each filing is subject to the general filing requirements of Rule R590-228, Submission of Credit Life and Credit Accident and Health Insurance Form and Rate Filings.(4) The commissioner may prohibit a form if the benefits provided are not reasonable in relation to the premium charged.Utah Admin.
