Section R510-104-13 - Home Delivered Meals(1) In addition to the eligibility criteria described in Section R510-104-4, an individual must be determined to be homebound to receive a home delivered meal.(2) An individual shall be determined to be homebound if the individual cannot leave home without assistance due to a disabling physical, emotional, or environmental condition. A determination of an individual's homebound status shall be made using a method of assessment approved by the division. (a) Any waiver of the assessment to determine an individual's homebound status shall be at the discretion of the local area agency.(b) A request for a waiver of the assessment to determine an individual's homebound status shall be submitted in writing to the local area agency's director or the local area agency director's designee.(c) A waiver request, and accompanying documentation, shall be maintained in the individual's client file.(d) An approved waiver request shall be reviewed on an annual basis.(3) A determination of an individual's homebound status shall be documented and maintained by the local area agency.(4) An individual's homebound status shall be reviewed or re-evaluated at least annually.(5) Each local area agency shall provide home delivered meals at least five days per week. A rural area where such frequency is not feasible may offer home delivered meals less frequently at the discretion of the division.Utah Admin. Code R510-104-13
