Section R501-8-8 - Staff Health Requirements(1) Prior to engaging in any field activity, each staff shall adhere to the following: (a) each field staff, intern, and volunteer shall have an annual physical examination and health history signed by a licensed medical professional;(b) a recognized physical stress assessment shall be completed as part of the physical examination of each staff;(c) the physical examination of each staff shall be reviewed and maintained by the provider in the staff personnel file; and(d) each program staff, intern, and volunteer shall submit to drug and alcohol screening upon request.Utah Admin.
