Section R436-5-6 - Legal Representative(1) A legal representative with authority to apply for registration of a new birth certificate is limited to any of the following: (a) the custodial parent;(b) both parents acting jointly;(c) a person given authority for purposes of making application for registration of the new birth certificate by a court of competent jurisdiction; and(d) a person granted power of attorney for purposes of making application for registration of the new birth certificate by: (i) the person who is the subject of the birth certificate, if of legal age;(ii) the custodial parent, if a sworn attestation of custodianship is included with the power of attorney; or(iii) both parents acting jointly.Utah Admin.
