(1) All forms, certificates, records, electronic data files, and supporting documentation used in the system of vital records and statistics are the property of the Department and are subject to the Department's rules.(2) Only forms furnished by the State Registrar shall be used in the reporting of vital statistics, creating records, or issuing certificates.Utah Admin.
