Section R432-700-12 - Contracts(1) The administrator shall secure written contracts or agreements from other providers, or independent contractors, who provide client services through the agency and shall arrange for an orientation to ensure that the contractor is prepared to meet the job expectations.(2) The licensee shall make any contract available for review by the department.(3) Each contract shall include: (a) the effective and expiration dates;(b) a description of goods or services to be provided; and(c) a copy of the contractor's professional license.Utah Admin.
