Section R380-350-4 - Notification of Expiring Certification(1) In accordance with Section 26-71-107, the department shall notify each state certified community health worker through the email on file with the Office of Health Equity when the certification is due for renewal.(2) The department shall inform certified community health workers via email six months before their certification expires with an additional reminder one month before their certification expires.(3) It shall be the responsibility of a certified community health worker to ensure that a valid and up to date email address is provided to the department through the application portal.
