(a) The forms of Appendix A have been adopted by the commission for use pursuant to the Act and this subchapter. Reproduction of these forms is authorized for use by applicants to complete the filings required.(b) The forms have been designated as follows:(2) application for quarry and pit safety certificate--Form-2114; Attached Graphic
