Section 844.25 - Complaint Requirements(a) A complaint must be filed in writing by the complainant completing the Complaint Form.(b) A Complaint Form may only be submitted by online submission as identified through the Agency's website page related to COVID-19 mandate complaints or by other means authorized in writing by the Agency.(c) The complainant must provide the following information on the Complaint Form: (1) the name of the complainant;(2) the name of the employer;(3) the nature and description of any alleged adverse action the employer took against the complainant; and(4) any other information specifically requested by the Agency on the Complaint Form that is necessary to resolve the complaint.(d) The complainant must declare that the information provided in the completed Complaint Form is true and correct.40 Tex.
