Section 550.802 - Coordination of Services(a) A center must adopt and enforce written policies and procedures regarding coordination of services to ensure the effective exchange of information, reporting, and coordination of a minor's services: (1) among all staff providing services on behalf of a center; and(2) between the center and a provider of services to the minor that is not providing services on behalf of the center, if known by the center.(b) Documentation in a minor's medical records must demonstrate coordination of services as described in subsection (a) of this section.(c) For a minor receiving services from a provider that is not providing services on behalf of a center, the center must:(1) not duplicate or provide services that conflict with a minor's care plan or service plan with the provider;(2) when requested by an adult minor or parent, make available a minor's records to support the coordination of services between the center and the provider;(3) request copies of a minor's records with the provider to support center care planning activities;(4) if requested by an adult minor or parent, participate in planning activities for a minor conducted by the provider;(5) request that a minor's provider participate as part of the center's interdisciplinary team and QAPI committee, as applicable; and(6) support the coordination of a minor's services by allowing a minor's provider to serve a minor at the center, if: (A) the center, a minor's parent, a minor, if the minor is an adult minor and the provider agree that the provision of services to a minor by the provider at the center would be appropriate for the minor; and(B) the center and the provider establish a written agreement for the provision of services at the center. The written agreement must include the provider's compliance with center policies and this chapter.26 Tex. Admin. Code § 550.802
