Section 1330-01-.16 - LICENSE(1) Issuance - Upon the Board determining that an applicant has successfully met all statutory and regulatory requirements, the Board shall direct the Division to issue the applicant a license in the classification for which he is qualified to practice.(2) Display of License - Every person licensed by the Board shall have on file a copy of his license in his office and, whenever required, exhibit such license to the Board or its authorized representatives.(3) Replacement License, or Renewal Document- A person's whose license, certificate, or renewal document has been lost or destroyed may be issued a replacement document upon receipt of a written request in the Board office. Such request shall be accompanied by an affidavit (signed and notarized) stating the facts concerning the loss or destruction of the original document and the required fee pursuant to rule 1330-01-.06. The damaged license, if available, must accompany the affidavit.(4) Verification of License - Requests for verification or endorsement of a license must be made in writing to the Board office and accompanied by the fee required by rule 1330-01-.06.Tenn.
