Section 1220-04-05-.08 - METER READING SHEETS OR CARDS(1) The meter reading sheets or cards shall show:(a) customer's name, address, and rate schedule;(b) identifying number and/or description of the meter (s);(c) meter readings;(d) if the reading has been estimated;(e) any applicable multiplier or constant.
