Section 1200-23-06-.09 - LOSS OF PERMIT OR LICENSE DOCUMENT(1) Any body piercing technician or establishment that loses, misplaces, or destroys a permit or license shall immediately apply for a duplicate, as soon as the fact becomes apparent. The fee for the duplicate shall be three dollars ($3.00). This fee shall accompany the application for such duplicate.Tenn. Comp. R. & Regs. 1200-23-06-.09
